How to Obtain Your Medical Marijuana ID Card

Determine eligibility and complete your registration

Though the use of medical marijuana has become more widely accepted in recent years, there are still strict qualifications that determine eligibility. Read below to see if you qualify for medical marijuana in New Jersey. If you are an eligible candidate, follow the instructions on how to register and obtain your ID Card.

Do you qualify?

The first step to acquiring your medical marijuana ID card is determining if you are a qualifying candidate. For a New Jersey resident to become a registered patient with the New Jersey Medical Marijuana Program, he/she must:

1.Establish a legitimate patient/physician relationship with a registered physician- the physician must be registered with the program and have a present and ongoing accountability for the evaluation and care of the patient's medical condition.

The physician can confirm the legitimacy of the patient/physician relationship by:

  • Maintaining the relationship for at least one year
  • Assessing/treating the patient's condition on at least four separate visits, or
  • Reviewing the patient's medical history, completing a physical exam of the patient and assuming responsibility for care of the patient's debilitating condition
2.Have a registered physician diagnose the debilitating condition - medical marijuana is only available to patients who suffer from certain debilitating medical conditions. In order to qualify for medical marijuana in New Jersey, the condition must be diagnosed by a physician who is registered with the MMP.

The following conditions are approved for the prescription of medical marijuana in New Jeresey:

-Amyotrophic lateral sclerosis
-Multiple sclerosis
-Terminal cancer
-Muscular dystrophy
-Inflammatory bowel disease, including Crohn's disease
-Terminal illness, if the physician has determined a prognosis of less than 12 months of life.

The following conditions apply, if resistant to, or if the patient is intolerant to, conventional therapy:
-Seizure disorder, including epilepsy
-Intractable skeletal muscular spasticity
-Post-Traumatic Stress Disorder

The following conditions apply, if severe or chronic pain, severe nausea or vomiting, cachexia or wasting syndrome results from the condition or treatment thereof:
-Positive status for human immunodeficiency virus
-Acquired immune deficiency syndrome

Additionally, medical studies have shown promise that marijuana can be beneficial for the following conditions, although, not yet approved for use in New Jersey:

- The treatment and prevention of Alzheimer's disease
-Anxiety/ Depression
-Reducing the symptoms of Auto-immune Disorders
-Sleep disorders
-Migraine headache
-Neuropathy/ Neuropathic pain syndromes

Complete your registration

If you feel that you meet the qualifications for obtaining an ID card for medical marijuana in the State of New Jersey, complete your registration online by following the steps below:

1.Go to and click "New Patient Registration"
2.Enter the required information, including the patient reference number provided by your physician
3.Select your preferred Alternative Treatment Center
4.Complete patient certification
5.Enter caregiver information, if applicable
6.Click "Save and Continue"
7.Upload the required documents

You will need the following documents to complete your registration. Caregivers, if applicable, must also submit these required documents.
Patient photograph - recent digital photo of patient. Photo must be against a white background and at least 70% of photo must display the patient's face. No glasses, hats, scarves or other items may cover any portion of the patient's face. The same conditions apply to submitted photographs of caretakers
Proof of identification - government-issued photo identification. Qualifying documents include:

  • Current NJ digital driver's license
  • Current NJ digital non-driver ID card
  • US passport, current or expired within past three years
  • Current US Passport card
  • Valid active duty US Military photo ID
  • US Military retiree photo ID
Proof of NJ residency - patients must live in the state of New Jersey to qualify. Patients can prove their residency by uploading a copy of:
  • Utility or credit card bill, issued within last 90 days, featuring name and current address
  • Bank/credit union statement issued within last 60 days
  • Original lease or rental agreement identifying you as the tenant
  • Property tax bill, statement or receipt from previous year
  • IRS or state tax office correspondence received within the past year
  • First-Class Mail received from and federal, state or local government within past six months
Proof of government assistance (optional) - you and your caregiver may qualify for a reduced application fee.

9.Download caregiver criminal background check form and follow the attached instructions

Once you have completed the registration process, the MMP will review your application and respond via email with the status of your application and further instructions.

If your application has been approved: the MMP will notify you of the fee amount in the email. Return to the MMP homepage ( and click on "New Patient/Caregiver Payment." Follow the instructions to make your e-payment. Once you have completed your payment, you will receive a confirmation number on the last page.

If your application has been denied: the MMP will state the reasons for denial and provide instructions to amend your application.
